NICE 2025/2026 Guideline Update (NG106)

NICE (National Institute for Health and Care Excellence) issued a critical update to the NG106 guidelines in mid-2025, which are now fully operational across the NHS. A major change in 2026 is that GPs can now initiate SGLT2 inhibitors and ARNIs on the advice of a specialist, whereas previously, these were often restricted to hospital-only starts.

This "GP-led initiation" is estimated to prevent 3,000 deaths and 5,500 hospital admissions annually in England. The goal is to get patients on their full medical regimen within weeks of diagnosis, rather than the previous average of 12–18 months.
